Insurance Billing Information

To the Parents/Guardians of University of Montevallo Student-Athletes:
 
On behalf of the physicians, athletic trainers, administrators, and staff in our Sports Medicine Department, we want to thank you for entrusting the health and well-being of your son or daughter to us.
 
The University of Montevallo files all medical claims for medical services provided within our facilities to your son or daughter with your primary insurance carrier first.  Any remaining balances that would normally be due by you, the member, will be paid on your behalf by University of Montevallo for injuries related to your son's or daughter's participation in athletic sanctioned practices and events.  All we ask is that you forward to us all communication from your insurance carrier (Explanation of Benefits [EOB], Request for Information, etc.).
 
With some of the recent changes in healthcare regulations, some providers used by the University of Montevallo could now be an "Out of Network" provider.  Normally, this does not mean much to you as the member; since the University of Montevallo covers the cost for your son's or daughter's medical expenses related to care received within our facilities.  However, some of these recent changes in regulations have caused carriers to send checks directly to the member rather than the provider who rendered the service(s).
 
If this should happen to you, we simply need you to follow the below steps:
  1. Sign the back of the check (Endorse the check)
  2. Send the check and any corresponding paperwork you received with the endorsed check to the University of Montevallo Athletic Department

The University of Montevallo will forward this check to the provider to clear out any balances due.  We cannot stress this next part enough… DO NOT CASH OR DEPOSIT THE CHECK.  Doing so could result in your athlete receiving what the NCAA describes as an "extra benefit", thus resulting in your son or daughter becoming INELIGIBLE TO PARTICIPATE in their respective sport.  Please contact Interim Head Athletic Trainer Maureen Wood if you have any questions.
